Making another wudhu without having done any act of ibaadat

Q: In Talim ul Haq Masaail pertaining to wudhu it says in number 13 - It is Mustahab (preferable) to make Wudhu for each Salaat even though one may be in the state of Wudhu. It is Mustahab only if at least two Rak'aats Salaat have been performed with the previous Wudhu. Thus if one did not perform any Salaat, it will not be permissible to perform fresh Wudhu before that Wudhu
has been either broken or at least two Rak'aats Salaat have been performed.

1-Is the above correct?

2-Will a person be sinful for doing fresh wudhu, if the wudhu is not broken or two Rakaats hasn't been performed because it says it will not be permissible to perform fresh Wudhu?

Doubts regarding making ghusal under a shower

Q: I made ghusl. I gargled a couple of times and put water on the nose couple of times then soaped the whole body once including the arm pits and under the private parts also inside the anus will the fard ghusl be done I can't remember if the bend near the anus got wet and the hairs on the anus got wet I was making ghusl under the shower so will the fard ghusl be done?

Scars

Q: A person has a rash like eczema on his scalp. Sometimes it bleeds a little. At night unconsciously he scratches it and it bleeds sometimes. Now it is very difficult to know where it bled etc, and at fajr time he simply makes wudhu as normal without washing his head. There may be some dry blood on his head. Is performing salaah in this manner permissible or does he have to wash his head completely every morning to make sure there is no blood? Sometimes for Fajr he has to be the imam of a small musallah when the Imam is not there?

Doubts when using a high pan toilet

Q: I use a raised toliet, when I use water to clean my private part, this water touches dirty water in the toliet then some dirty tiny drops of water bounce back to my legs so basically when I was wash my private part with water this water drop down and mix with dirty water then bounce back to me.  When I look carefully I see very tiny drops of water on my legs. Do I need to wash my legs with clean water? I was told by an Aalim that I don't need to wash my legs because they are only tiny drops of water.  I was told to ignore this but I worry my salah will not be valid.

Doubts with regards to ghusal

Q: I made ghusl and soaped my body twice then gargled and put water on my nose but something tells me I've missed places out although I soaped the whole body twice. Could it be possible for me to miss a place out if I soaped the whole body twice and will fard ghusl be done?
