When should miswaak be made in wudhu?

Q: When should khilaal of the fingers be made in wudhu? Should it be made after washing the arms till the elbows or after making masah of the head?
Q: A person did not wash a part of his body properly in wudhu, or forgot to wash a part completely. He finds this out after he has completed his wudhu and dried himself. Can he just wash that part again to make his wudhu valid or does he have to repeat his wudhu?
Q: I used something called a douche... I insert it with liquid medication and insert it into my private part and wash myself out. Thereafter I make wudhu. When I come out of the shower, some of the water comes out from my private part. Is my wudhu still valid?

1. If I saw some whitness on my underwear and doubt whether it is impurity or not, does this break my wudhu?
2. What if I have prayed before seeing the whiteness, is my prayer valid?
