Semen coming out with desire and force

Q: I need clarification on this issue. I would appreciate a detailed response if that is ok. Does mere orgasm in a man or woman require ghusl? I was studying fiqh [quduri] with a brother and he said even if you have an orgasm and nothing comes out ghusl will be required. His reasoning was the text mentions leaving its place of origin [i.e. the loins] and when you orgasm this sperm leaves its place of origin. I further asked him what if you took 40 steps or slept or urinated and then sperm came out without desire, he said even then ghusl required as it is stuck in the passageway and will come out with urination. But sunnipath says otherwise:

In the Hanafi school, in order for the ejaculation to nullify ritual purity, it must be accompanied with sexual gratification (shah-wah) at the time of leaving it's place of origin (i.e. the loins). Once orgasm has begun and based on it ejaculate exists from the penis, ritual purification will be nullified. According to the stronger opinion within the school, forceful exiting in spurts is not a condition provided that the initial cause was orgasm and sexual gratification (and not lifting some heavy object etc.). Given this, semen which remained in the passageway and exited after the completion of ghusl will require that the ghusl be repeated. However, when sleep, urinating or excessive walking occurs after the bath and before the exiting of the semen, due to these movements cleaning the passageway and halting the dripping of all semen, any semen to exit thereafter will not be attributed to the initial gratification. In the latter case, owing to absence of sexual gratification, ritual purity will remain intact. [Ibn Abidin, Radd al-Muhtar, 1.160]

Is there a difference of opinion?

Retrograde ejaculation

Q: Because of medication, I experience retrograde ejaculation. When I reach an orgasm during sleep (due to a dream), nothing comes out, since the sperm is redirected to the urinary bladder and comes out later, with the urine. Does such a "wet dream" necessitate ghusl even though it isn't wet in my case?

Ghusal

Q: Does merely inserting your penis in between buttocks make ghusl obligatory? It did not go inside the anus, just touched the outside of it.

Wet dream

Q: The underwear I had on I cant like really see any round patch on it so should I just consider my body paak and consider ghusl to not be fard?

Doubting if mani was discharged

Q: Okay I have a question.. and I really do need specific instructions/what to do; please do give a descriptive answer to clear my mind of any doubts.. 

[This is from what I can remember because it was a while ago] 

So the day before I believe I had a 'nocturnal emission' [wet dream] and purified myself. Then the night before I wore a pair of bottoms that I hadn't worn in a while. When I woke up I believe it was fajr time, [can't remember if I had a dream or not] entered the toilet and noticed whitish like tiny patches that were not concentrated in one spot [as it would be normally] nor was the surface of the bottoms to my knowledge 'crusty'. Also it was dry, not wet or damp.. I think the bottoms still smelt clean [Might of been washed before I wore it]. So after thinking about it to myself for a while. [I did have doubts before to do with wudoo' [waswaas etc. but rarely or never before with emissions] I changed, did wudoo' and thinking that it was stains/white specks from a while ago I prayed thinking that I was okay and not in need of wudoo'.. I did have doubts though and later, [after fajr I believe] had a shower [just a normal shower I believe] Now I really don't know and I am confused.. It I was definitely sure I needed ghusl I would repeat the salah... but I just don't know and I am worried about what to do... Please help me
