Doubts in Salaah

Q: If one has frequently doubts in salaah, that he is recording his salaah with his audio-record function of his phone to check afterwards whether he prayed correctly if some doubt arise about something. Now say he is praying Zuhr and in the last rakaat when he sits after the sajdah, he does not not know whether this was from the 1st sajdah or 2nd sajdah i.e. he cannot totally remember that he did another sajdah before it, but continues on this basis (because he can check with the audio-record anyway) and finishes his prayer. Now afterwards when he checks his audio-record it shows that he did 2 sajdahs thus prayed correctly. Is the salaah valid ?

Q: I have OCD therefore in salaah I try to keep silent but as I try to do so it counter attacks me (the opposite occurs), thus by force I make a small sound or a small sound which can be the result speech, thus I should repeat my salaah until I feel I havent made a sound or speech, but lately I said enough is enoug this can be something like wasaawis which I also have a problem with thus weather I make a small sound or speech I regard my salaah as valid, please what should I do?

Q: What should be done if one forgets the number of rakaats completed while reading Salah?

Q:

1. If I felt like a drop of urine came out during salaah but I was not certain, after salaah I saw that a drop came out I do not know if it was during salaah. Is my salaah valid?
2. I was also not certain if it came onto my clothes and regarded it as pure. Is this correct?

Q: If one has a doubt if he missed a waajib or fardh act what should he do?

Q: If one has a habit of forgetting multiple things in a Salaah such as number of rakat, number of sajda, first tashahud and so forth, how that person would perform his salah?

Q: When I do a long sajdah I sometimes forget if it is the first sajdah or the second sajdah. What should I do in this situation?
