Making sajdah on one's forehead only

Q: If I performed sajdah only on my forehead, and not on my nose, is it necessary for me to make sajdat al-sahw? Do I have to repeat my salah afterwards?

A: The sajdah will be valid through only making sajdah on your forehead and not your nose.

Though the sajdah is valid, however this is against the sunnah and it will affect the reward of the salaah if done without a valid reason. An example of a valid reason is not making sajdah on the nose due to a wound on the nose or some other valid medical reason.

Therefore, since your sajdah is valid, your salaah is also valid and sajdah sahw is not compulsory.

And Allah Ta'ala (الله تعالى) knows best.
