Commencing salaah with Surah Fatiha

Q: I read in tafseer Ibn kathir that Surah Fateha has seven verses in it and there are some Hadith also there in that tafseer from two sahihs that Bismillahir rahman nir Raheem is a first verse of Surah Fateha. There are only seven verses in Surah Fateha. Now, I saw in Sahih Bukhari Narrated From Anas bin Malik (radhiyallahu anhu): Nabi (sallallahu alaihi wasallam), Abu Bakr (radhiyallahu anhu) and 'Umar (radhiyallahu anhu) used to start the prayer with "Alhamdulillahi rabbil aala min" Vol-1,book 12, number 710. ( Sahih Bukhari-- characteristics of prayer). Now, what should we start the prayer with?

Q:

1. Is it compulsory to recite Tasmiyyah before Surah Fatiha in the 2nd, 3rd & 4th rakaats of Salaah.

2. If we mistakenly recite Surah Naas in the first rakaat of Salaah, how do we continue with the rest of Rakaats? Will Sajda-e-Sahw be compulsory?

3. If I mistakenly recite a Surah in the second rakaat which is earlier in order to what was recited in the first rakaat, should an additional latter Surah be recited if it is realised within the rakaat?

Reciting a surah or aayaat equivalent to a surah in Salaah

Q: I believe that in a book I read titled, Laws of salah, written by Muhammad Ilyas Attar Qadari Razavi (Dawat-e-Islami) it said on the hundred and fifth page these following words:-

'After you finish surah Fatihah, utter amin in a low volume and then recite either three short ayahs or one long ayah that is equivalent to three short ayahs or any surah such as surah Ikhlas.'

I had a query based on the last line of this paragraph. When it quotes, 'or any surah,' do you think that this is implying that after surah Fatihah has been recited you can recite another surah, or is it suggesting that after surah fatihah you can recite an ayah that is equivalent to another surah, or do you think that it can be referring to both?
